General Price List

These prices are effective as of December 1, 2007 and are subject to change without notice.

The goods and services shown below are those we can provide to our customer. You may choose only the items you desire. (However, any funeral arrangements you select will include a charge for our basic services and overhead.) If legal or other requirements mean you must buy any items you did not specially ask for, we will explain the reason in writing on the Statement we provide describing the funeral goods and services you selected.

TRADITIONAL FUNERAL SERVICE SELECTION: ..... $3,985.00
This includes services of the funeral director and staff, transfer of remains to funeral home/within 25 miles, embalming, other preparation of the deceased, use of facilities for viewing, (1) day and funeral ceremony or service in other facility, funeral coach, funeral service escort, flower vehicle, committal or other disposition service, acknowledgment cards, register book, memorial folders/prayer cards.

The following * costs are included in the above price, you may choose any additional services and merchandise that you desire. Cremations prices are also listed below.

*BASIC SERVICE OF FUNERAL DIRECTOR AND STAFF ..... $1,275.00
Our charges include consultation with family or responsible party; staff and facilities to respond to initial request of service; coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased; securing official documents and sheltering of the deceased. This fee is for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremation, immediate burials and forwarding or receiving of remains.)

*EMBALMING ..... $650.00
Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements such as funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it such as direct cremation or immediate burial.

*TRANSFERRING REMAINS TO THE FUNERAL HOME ..... $250.00
A mileage fee of $1.50 per loaded mile will be added to the charge listed for each mile beyond our local service area. (Within a twenty-five (25) mile radius of Whiteland, Indiana)

VISITATION AT OTHER FACILITY: Our service will include local transportation of encased remains, visitation setup, all necessary equipment, placement of floral tributes and supervision and attendance during visitation for one (1) day or any portion of a day.

Alternative Disposition

Our charge for an immediate burial (without ceremony or viewing) includes:  Blue or beige cloth-covered casket, concrete outer burial container, removal from place of death and transportation to cemetery..... $2,405.00

Immediate Burial With Services Includes:  Removal of deceased to funeral home, basic services or funeral director, embalming, necessary authorizations, permits and transportation to local cemetery, use of facilities and staff for visitation and funeral ceremony for 2 hours prior to burial ..... $2,950.00

Immediate Burial with casket & out burial container provided by purchaser ..... $1,275.00

Immediate Burial with any casket & outer burial container is this fee plus the casket selected ..... $1,275.00


DIRECT CREMATIONS available from ..... $1,350.00
Our charge for a direct cremation (without ceremony) includes: Alternative container, removal of deceased to funeral home, sanitary procedures permitting positive identification, basic services of staff, necessary authorizations and permits, transportation to crematory, cremation fee and return of cremated remains in a temporary container.

If you want to arrange a direct cremation, you may use an alternative container. Alternative containers encase the body and may be made of materials like fiberboard or composition materials (with or without covering.) The containers we provide are Cardboard Alternative Container or any Combustible Casket.

Please Note: Local crematory requirements prohibit the use of non-rigid containers, container must be of combustible material.

Transfer Arrangements
FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME ..... $550.00
This charge includes: Local removal to our funeral home, basic services of the funeral director and staff, necessary authorizations and permits, embalming, local transportation (except airline or other shipping charges). This charge does not include visitation, rites or ceremonies prior to forwarding of the deceased. The purchase of a casket and/or shipping container is additional.

RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME ..... $1,185.00
This charge includes: Local transfer to our funeral home, basic services of funeral director and staff, care and custody of deceased and hearse to local cemetery or crematory. Other services such as facilities and staff for visitation or ceremony and other transportation may be selected from the GENERAL PRICE LIST as additional charges. A mileage fee of $1.50 per loaded mile will be added to the charge listed for each item of motor equipment used beyond our local service area.

TERMS OF PAYMENT AGREEMENT
Payment in full is required prior to funeral services being rendered. We honor cash, check, Visa, Mastercard, Discover, American Express and insurance policies.


INSURANCE ASSIGNMENTS
We will honor an insurance policy/assignment to be completed at the time of the arrangement conference. The policy must be provided to us by the date of the funeral service. There will be 3% processing fee for all insurance assignments, to be paid in advance. All in insurance policies/assignments are processed by our Funeral Funding Center.
